Patient Registration Forms
If your insurance has changed you need to complete the Patient Registration Form for patients with Commercial Insurance. You may fax the form to us, along with a copy of your new insurance card (front and back), to 770-461-8852. Please bring in the original form and your insurance card to your next visit.

Athletic Forms
There is a standard form used for clearance for athletic activities in Georgia. It has been adopted by the Georgia High School Association for use in all Georgia schools. You must fill out the history portion and bring it with you to your sports physical, and we will complete the form.
